Celebrating the Power of Collaboration: Our Care Team & Collaborative Care Program

At Samaritan Community Center, we believe that when we work together, we can achieve remarkable things. This spirit of partnership is the heart of our Collaborative Care Program, and we are incredibly proud of what we’ve been able to accomplish by teaming up with other local nonprofits. Together, we’re making it easier for struggling families to access the resources they need, all in one place.

A shining example of this collaboration is our work with The Family Network and The Diaper Collective. These organizations have provided us with free diapers all year long, which we distribute through our Samaritan Market. For many families, diapers are an ongoing expense that’s difficult to keep up with, and receiving this essential item at no cost has made a tremendous difference in their lives. In fact, we’re honored to announce that The Diaper Collective has just awarded us their prestigious "One Millionth Diaper" award, celebrating the millionth diaper they’ve given out in

partnership with us. This incredible milestone speaks to the power of collaboration and the impact we can make when we come together to support a common mission.

But this is just one example of how our Collaborative Care Program is changing lives. Through partnerships like this one, we’re able to break down many of the barriers that our clients face. Transportation is often a major obstacle—many of our neighbors lack reliable transportation, making it difficult for them to access multiple services in different locations. By bringing nonprofits and their services into our building, we create a one-stop-shop where families can get diapers, food, clothing vouchers, and more, all in one visit. This saves time, reduces stress, and helps ensure that people are getting the holistic support they need to move forward.

We’re deeply grateful to The Diaper Collective, The Family Network, and all of our nonprofit partners for their continued support in making our Collaborative Care Program such a success. Thanks to their help, we can provide the highest quality of care to our clients while making sure that no one has to face their challenges alone.
